Federal law allows you to change your benefits only when you experience a qualifying life event (also called a change in status). These events include:

• Marriage or divorce, legal separation, annulment, or the death of a spouse.
• Changes to your dependents, such as a birth, adoption, placement for adoption, or the death of a dependent.
• Employment changes for you, your spouse, or your dependent—such as starting or ending a job, a strike or lockout, changing your worksite, or any other job change that affects benefit eligibility.
• When a dependent gains or loses eligibility for coverage—such as aging out, changes in student status, or similar circumstances.
• A change in residence for you, your spouse, or your dependent that affects your coverage—such as moving out of an insurance plan’s service area.

Qualifying Life Events generally must be submitted within 30 days of the date of the event (or 60 days after gain/loss of Medicaid or State healthcare coverage).

The only exception is for birth, adoption, or placement for adoption—in those situations, coverage begins on the date of the event, as long as you notify the Plan within the 30‑day window.

To process your change in Workday, you must also provide supporting documentation that verifies the event. The documentation required to submit a change for a Qualifying Life Event (QLE) is as follows:

• Marriage: the official marriage certificate issued by state/county/city in which you were married
• Divorce: a copy of the official divorce decree
• Birth: a copy of the birth certificate
• Adoption: a copy of the adoption agreement
• Loss of Coverage: a letter showing the last day of benefit coverage through the original source
• Gain of Coverage: a letter showing the first day of coverage through new source
• Death of a Dependent: the official death certificate
